Community Pharmacy Assurance Framework (CPAF) screening questionnaire 2025/26

Completing the 2025/26 Community Pharmacy Assurance Framework (CPAF) is mandatory.

The questionnaire, open until Thursday 31st July 2025, must be completed as part of pharmacy owners’ Terms of Service requirements. Later, NHS England will select a small number of pharmacies for a monitoring visit and/or to complete the full CPAF questionnaire. The screening questionnaire has just 10 questions and can be answered online in around 10-20 minutes. We recommend that pharmacy owners complete the questionnaire in good time before the deadline.  For more information, visit the CPE website.
