Reporting Controlled Drug (CD) Incidents and Concerns

Prompt reporting of incidents and concerns about any schedule of CD is crucial to prevent recurrences and patient harm. This guide has information that explains what to report, how and who to. Please ensure key staff are aware of the CD reporting address www.cdreporting.co.uk

To help identify forged or fraudulent prescriptions Pharmacies are encouraged to check the validity of all paper NHS and private prescriptions, especially those that have been handwritten, by:

  • Checking someone’s identity before signing for a prescription
  • Seeking consent from the patient to access their Summary Care Record to check the validity of the prescription
  • Contacting the prescriber if consent is not given, or it is not possible to gain consent for some other reason to access their Summary Care Record
  • Making notes of person presenting prescription – save any CCTV

And appropriately notifying:
•    Police 101 (non-urgent), 999 (immediate)
