Patient Stories

Community pharmacies make a real difference to patients every day. We want to share stories that showcase the positive impact pharmacy teams have on patient care and local communities.

Share your story

If your pharmacy has a patient story that demonstrates the difference community pharmacy has made, we would love to hear from you.

Please email your patient story to info@cpherts.org.uk.

Before submitting a story, please ensure:

  • You have obtained the patient’s consent to share their story.
  • All patient-identifiable information has been removed or anonymised.
  • The patient understands their story may be shared through CPH communications, including the CPH website, newsletters and social media channels.

What makes a great patient story?

The best stories:

  • Focus on a patient’s experience or a pharmacy team member’s contribution.
  • Show how pharmacy helped improve care or access to services.
  • Highlight the difference pharmacy makes to patients and communities.
  • Include patient quotes where possible (with consent).
  • Are clear, positive and concise.

Please include:

  • What happened?
  • What did the pharmacy team do?
  • How did this help the patient?
  • What difference did it make to the patient, the NHS or the wider community?

Stories can be submitted in writing or as a short video, where appropriate and with the correct consent.

Thank you for helping us showcase the important role of community pharmacy in supporting patients across Hertfordshire
