GP Connect in Practice – Community Pharmacy Quick Guide

GP Connect supports safer, more joined-up care by allowing pharmacy teams to both view and share patient information with GP practices during NHS service delivery.

GP Connect Access Record (View GP Information)

What it does: Allows you to view key patient information from the GP record during a consultation.

What you can see:

  • Current and recent medications
  • Allergies and adverse reactions
  • Relevant clinical history (limited view)

When to use it:

  • Pharmacy First consultations
  • CPCS referrals (NHS 111 / GP)
  • Emergency supply requests
  • Any consultation where clinical history supports safer decision-making

How to access:

  1. Open the patient consultation in your clinical system
  2. Select “GP Connect” / “View GP Record”
  3. Confirm patient consent
  4. Review information
  5. Use to support your clinical decision
  6. Record that access was used

 

GP Connect Update Record (Send Information to GP)

What it does: Allows pharmacy consultation outcomes to be shared back to the GP practice.

What is shared:

  • Consultation outcome
  • Medicines supplied
  • Advice given
  • Referrals or escalation

When it is used:

  • Pharmacy First consultations
  • CPCS outcomes
  • When GP follow-up is required
  • Where services are commissioned to send updates

How it works

  • Completed automatically through your system when you finish and submit a consultation
  • No separate login required
  • Sent as a structured update into the GP record

Note on Consultation Records

Information entered into your clinical system may be shared directly with the patient’s GP practice via GP Connect Update Record.

It is therefore important that consultation notes are clear, accurate, and clinically relevant, as they will form part of the patient’s GP record.

Ensure that entries:

  • Clearly describe the presenting complaint and assessment
  • Record any medicines supplied or advice given
  • Include relevant clinical reasoning and outcomes

High-quality consultation records support continuity of care, patient safety, and effective communication with GP practices.
