Reducing Harm from Medicines Drop-in session
HWE ICB are holding drop-in sessions to support clinicians to conduct polypharmacy reviews and deprescribe where clinically appropriate. Sessions will usually start with approximately 15 minutes of ‘micro-teaching’ supported by a clinician although some sessions will have a greater taught element. HWE ICB also encourage sharing of successes to facilitate learning.
The next drop-in session to support deprescribing is scheduled for Monday 18 November 12.30-1.30pm and the topic for this session will be ‘High risk patient review – Linking Eclipse Live with IMPACT review’. This session will be delivered by Shirley Ip, Lead Frailty Pharmacist, PAH and HWE ICB.
