CPH are aware that a lot of surveys and feedback questionnaires are offered to pharmacies and it can be difficult sometimes to find the one you want, when you want it or even find the time to complete them.
In an effort to make this easier, we have set up this page which will hold surveys and questionnaires in one easy to access space. If you’re looking for one and can’t find it on here, please contact us at info@cpherts.org.uk
